You can download a PDF registration form here.  You will receive your information packet for the patient workup in a few days–please let Dr. Engelberg know if you have any questions about the materials, photographs needed, or impressions of your patient.

Tuition:

  • Doctors: $4,690
  • First Dental Assistant: $545
  • Each Additional Staff Member: $185
  • A dentist can audit the course with no patient for $2,800 total.
  • Returning dentists can take the course at a 30% discount.
  • Lab fees are separate and are paid directly to Brad Jones and Smiles, Inc.

**$4,690 for dentists, $545 for first dental assistant, and $185 for each additional team member**
**Dentists can audit the course (with no patient) for $3,800 total plus $300 per staff member**
**If pre-paid in full prior to September 15th, your tuition is less 5%**
**Returning dentists tuition is less 30%**

*A 30-day advance notice of cancellation is required for a refund less the registration fee of $200 per attendee. For cancellation less than 30-days, the course fee may be applied to a future seminar.
